Role Overview

We are seeking a WMO Consultant to support residents within the framework of the Social Support Act (Wmo). In this role, you will conduct home visits and structured interviews with residents to assess their support needs and determine appropriate solutions that enhance self-reliance, participation, and independent living.

You will work within the social domain to ensure residents receive tailored support while balancing legal frameworks, available services, and individual circumstances.

Key Responsibilities

Client Assessment & Home Visits

  • Conduct home visits and kitchen table conversations with residents
  • Assess personal situations and support needs in a structured and empathetic manner
  • Explore available support options within the resident's own network and community resources
  • Analyse complex social situations and determine appropriate interventions

Support Planning & Reporting

  • Prepare formal support plans in line with Wmo legislation
  • Draft reports, decisions, and administrative documentation accurately and timely
  • Ensure decisions are well-substantiated and compliant with applicable laws and regulations

Coordination & Stakeholder Engagement

  • Collaborate with healthcare providers, social partners, and internal municipal teams
  • Monitor the quality and progress of provided support services
  • Adjust support plans when necessary based on changing circumstances
  • Act as a connector between residents and relevant service providers

Case Management

  • Manage multiple cases simultaneously in an organised and structured manner
  • Ensure accurate administrative handling of all assigned files
  • Maintain high standards of confidentiality and compliance throughout the process

Assignment Deliverables

  • Well-founded Wmo assessments and decisions for residents
  • Structured and compliant support plans
  • Accurate and complete case documentation
  • Effective coordination with care providers and social domain partners
  • Improved resident self-reliance and participation outcomes

Required Qualifications

  • Completed HBO degree in Social Work, Social Legal Services, or a related field
  • Minimum 6 years of experience as a Wmo Consultant or similar role within the social domain
  • Strong knowledge of the Social Support Act (Wmo) and related legislation
  • Experience conducting home visits and structured resident interviews
  • Ability to draft support plans, reports, and formal decisions
  • Excellent Dutch communication skills (written and spoken)
  • Must be eligible to pass pre-employment screening
  • Valid identification document required at start and intake

Preferred Experience

  • Experience with mental health care and sheltered housing
  • Knowledge of home adaptations, mobility aids, and assistive devices
  • Experience working within municipal social services
  • Familiarity with complex social care cases and multi-agency collaboration
